Patient Online

 Giving you more choice in accessing GP services

 What is patient online?

Patient online will help you to take greater control of your health and wellbeing by increasing online access to services.

 You can now access the following services online:

  • Book Appointments
  • Renew Prescription
  • View your GP record

To access these services online you will need to collect an application form from the surgery. Complete, sign and bring this to the surgery together with 2 forms of identification (one with a photograph and one recent document showing your current address). If you have any difficulty with this please speak to one of our receptionist.  Your request will then be processed and registration details will be sent to you in the post.

The shingles vaccine is now available to all those aged 70 or 79 years.

Non NHS Medical Services and Examinations

steth_book1The National Health Service does not cover some of the services you may be required and you may be requested to pay the doctor a fee which is recommended by the British Medical Association. A list of all these fees is available at the reception desk. 

  • Private sick note 
  • Private Insurance form
  • Private medical reports and examinations
  • Holiday cancellation forms
  • Insurance claim forms
  • Fitness to travel form/letter
  • Holiday vaccinations such as Japanese Encephalitis, Tick-borne Encephalitis, Rabies and Hepatitis B 

If you require a private medical examination such as HGV or PSV, please inform the
receptionist that you need to book an appointment for a medical as these appointments
are longer appointments at certain times of the afternoon only.
