From 31st March 2015 your 'Summary Care Record' which includes allergies, adverse reactions and medication details will be uploaded to the NHS spine so that out of hours, A&E and the Ambulance Service can access these important details about you. If you do not want your details shared please contact the Surgery.

The data shared is only demographic (address and date of birth etc.) and information about medication and allergies. This information can only be seen by people who are Health Care Staff who are DIRECTLY INVOLVED in your care.
You can opt out at any time and you can refuse permission for any Health Care Staff to view the information (they must ask you each time).
At BPC we consider confidentiality our utmost priority. At present we believe the Summary Care Records is secure and of benefit to our patients. However we will continue to monitor the situation and will not allow our patients confidentiality to be put at risk.
Your Summary Care Record will be created and shared automatically unless you opt-out.

If you have already told us you do not want to share your information then we will honour this. You may however want to fill in an opt-out form, as well as we find people some times change their minds.
