Please do not attend your appointment if you have symptoms of covid-19. To protect our vulnerable patients and our staff, please wear a mask for the duration of your appointment.
Please arrive as close to your appointment time as possible and let us know if you need to reschedule your appointment so we can offer it to someone else.

It would help our admin team if you brought your NHS Number with you. Please click here to find out what yours is if you don't already have it. It can also be found on the top of any hospital letters or via your NHS App.
Pre-screening Questions for Flu and Covid Vaccinations
Please take a look at the questions your clinician may ask you before your vaccination appointment.

Has the individual ever had any serious allergic reaction to any ingredients of the seasonal flu vaccines, drug or other vaccine? Is the individual currently unwell with fever? Does the individual have an allergy to egg? |
Has the individual experienced major venous and/or arterial thrombosis occurring with thrombocytopenia following vaccination with any COVID-19 vaccine? Has the individual had any vaccination in the last 7 days? Is the individual currently unwell with fever? Has the individual ever had any serious allergic reaction to any ingredients of the Covid-19 vaccines, drug or other vaccine? Has the individual ever had an unexplained anaphylaxis reaction? Does the individual have a history of heparin-induced thrombocytopenia and thrombosis (HITT or HIT type 2)? Does the individual have a history of capillary leak syndrome? Has the individual indicated they are, or could be pregnant? Has the individual informed you they are currently or have been in a trial of a potential coronavirus vaccine? Is the individual taking anticoagulant medication, or do they have a bleeding disorder? Does the individual currently have any symptoms of Covid -19 infection? |
