Care Management Service
We are excited to be involved in a new project working with local GP practices in West Ipswich and the GP Fed.
The new care management service will focus on a small group of our patients and it is a partnership with community nursing and social care.
The dedicated team will share information about you and work closely with you, your family &/or carers to keep you at home rather than in hospital unless you absolutely need to be there.
